10 questions to ask before you commit
- Are you licensed by the state, and accredited by The Joint Commission or CARF?
- What does your typical day look like? How many hours of actual clinical work?
- Who runs my care — a physician, a nurse practitioner, a counselor? What's their experience with opioid-type dependence?
- Do you offer medication-assisted treatment (buprenorphine-based) if a clinician thinks it fits me?
- What's the patient-to-staff ratio in detox? On the unit?
- How do you handle co-occurring mental health conditions like anxiety, depression, or trauma?
- What does the step-down plan look like after detox or residential?
- What's included in the price? What's billed separately (labs, medication, therapy add-ons)?
- What happens if I want to leave early?
- Can I talk to an alumni — someone who actually completed your program?
Red flags — walk away
- Pressure to commit the same day you call, especially before insurance is verified.
- Refusal to share licensing or accreditation in writing.
- Claims of a "guaranteed cure" or extremely high success rates with no source.
- All staff are "counselors" — no licensed medical or clinical leadership.
- Big push to fly you across the country before considering anything local.
- No clear step-down plan after detox.
- Refusal to put pricing or expected out-of-pocket cost in writing.
